Job summary

ForHealth Medical Centres in Bankstown is seeking a permanent full-time Chronic Disease Management (CDM) Nurse to lead local preventative health efforts and support GPs in managing chronic conditions, helping patients stay out of hospital.

You will prepare care plans, empower patients, and coordinate multidisciplinary care while conducting health assessments and ensuring timely reviews. This role emphasizes patient-centred care and community health impact.

Qualifications

• The candidate is an AHPRA-registered nurse with chronic disease management experience.
• Experience with health assessments is preferred.
• Strong communication and organisational abilities required.

Responsibilities

• Prepare Care Plans and Team Care Arrangements for patients.
• Empower patients to take ownership of their health.
• Coach patients on embedding health behaviour change principles.
• Coordinate access to ongoing care from a multidisciplinary team.
• Assist in early identification of chronic disease via health assessments.
• Support GPs to promote preventative care in the practice.
• Increase the number of patients receiving a care plan for the first time.
• Ensure care plans are reviewed regularly and complete related admin tasks.

The Role

We are looking for a Permanent Full-Time experienced Chronic Disease Management (CDM) Nurse to join our Bankstown practice to be the driving force and advocate for the local community in preventative health. This role will see you support GP's and their patients to identify and manage their chronic conditions and ultimately keep them out of hospital.

The Tasks

• Prepare Care Plans and Team Care Arrangements to help patients manage their disease/s
• Empower patients to increase ownership over their health
• Coach patients on embedding health behaviour change principles
• Enable patients to access ongoing care from a multi-disciplinary team
• Assist in early identification of chronic disease by conducting health assessments
• Work with GPs to promote preventative healthcare throughout the practice
• Increase the number of patients who receive a care plan for the first time
• Ensure patient's care plans are reviewed regularly
• Manage administrative tasks relating to carrying out CDM appointment

The Successful Candidate

• AHPRA registered Enrolled or Registered Nurse
• Proven prior experience in Chronic Disease Management and health assessments
• Knowledge of Health Assessment is highly regarded
• Strong organisational and time management skills
• Outstanding communication and customer service skills
