Job summary

CatholicCare NT is seeking a Case Worker (Crisis Accommodation) to join the Crerar Road facility, providing intake and case management for individuals and families in crisis accommodation.

Working with a multidisciplinary team, you will identify crisis needs, develop housing stability plans, and support social cohesion on site. This is a permanent full-time role, Friday to Monday, with a commitment to Aboriginal communities and wellbeing.

Qualifications

• Diploma or degree in Social Work, Community Services or similar.
• Experience in homelessness, housing, or crisis casework; knowledge of Aboriginal communities in the NT is highly regarded.
• Strong case management, assessment and planning skills.
• Demonstrated ability to work collaboratively in a diverse, multi-disciplinary team.

Responsibilities

• Deliver intake and case management for individuals and families in crisis accommodation.
• Identify crisis presentations and develop action plans to support housing stability.
• Support social cohesion through on-site activities and coordinated services.
• Collaborate with Housing Support Workers and Family Support Worker to improve outcomes.

We provide a diverse range of social services that support individuals, families, children, schools, and communities, many of whom live in remote or very remote areas. Our Crerar Road accommodation facility provides predominately an Indigenous accommodation service, comprising of a 190-bed shared visitor accommodation over two adjacent properties. The Crerar Road accommodation facility provides a home away from home experience to our participants who come to town for medical appointments, shopping, catching up with family or sporting events. In addition to this we provide renal, and homeless accommodation.

About the Role

The Case Worker (Crisis Accommodation) plays an important role within the multi-disciplinary team at CatholicCare NT's Crerar Road facility, delivering high quality, participant centered intake and case management for individuals and families in Crerar Road who are experiencing crisis accommodation needs.

Working alongside the Managed Visitor Accommodation Service, Housing Support Workers, and Family Support Worker, the Caseworker proactively identifies crisis presentations, develops action plans to support housing stability, and supports social cohesion through support activities on site.

This position is a permanent full time opportunity working a compressed week, Friday - Monday.

What we are looking for

• Diploma or degree in Social Work, Community Services or similar (or substantial progress towards qualification)
• Experience in homelessness, housing, or crisis casework; and knowledge of issues affecting Aboriginal communities in the NT is highly regarded
• Strong case management, assessment and planning skills
• Demonstrated ability to work collaboratively in a diverse, multi-disciplinary team
