Job summary

NSW Health invites applications for a paediatric case manager to support children with complex physical and cognitive disabilities within Kids Rehab, NSW. The role focuses on developing and implementing rehabilitation plans in collaboration with a multidisciplinary team.

Based at Westmead, you will liaise with families, schools and community clinicians, contributing to teaching, research and quality assurance activities within rehab and the SCHN.

Responsibilities

• Provide case management to children with complex physical and cognitive disabilities who are clients of rehab.
• Collaborate with the treating team to develop and implement rehabilitation plans.
• Undertake outreach liaison with families, schools and community-based clinicians and services.

Remuneration:121568 - 125507 full-time equivalent base salary range (+12% super + 17.5% leave loading and salary packaging)

Kids Rehab is the largest tertiary paediatric rehabilitation service in NSW, caring for children who as a result of an injury, illness, health condition or surgical intervention would benefit from a multidisciplinary program of goal-directed, medically-led care.

What you'll be doing

The case manager will provide case management to children with complex physical and cognitive disabilities, who are clients of rehab. The case manager, in collaboration with the treating team, develops and implements rehabilitation plans and undertakes outreach liaison with families, schools, community based clinicians and services.
Participation in teaching, research and quality assurance activities within rehab and the SCHN is essential.
