Job summary

Crescent Care Foundation is seeking a Case Manager to coordinate case management for people with traumatic brain injury, spinal cord injury and other severe injuries, from hospital discharge to community living in Melbourne. You will work in a person-centred way, plan goals, coordinate rehab and services, assess risk, and prepare plans under Lifetime Care and workers compensation.

Requires degree in related field and 5+ years experience, 20 hours/week, Ahpra if required, police and working with

Qualifications

• Bachelor’s degree or higher in Occupational Therapy, Social Work, Nursing, Psychology, Physiotherapy, Speech Pathology, Exercise Physiology, Rehabilitation Counselling, or another health, rehabilitation, disability or human services discipline accepted by icare.
• At least 5 years of full-time (or equivalent) professional experience in person-centred case management for people with severe injuries, disabilities and complex needs.
• Availability for at least 20 hours a week across 4 days during business hours.
• Current Ahpra registration, if your profession requires it.
• A National Police Check, and a Working With Children Check if you will work with people under 18.
