Crescent Care Foundation is a Liverpool-based NDIS-registered provider and ACNC-registered charity. We support people with disability and their families across South Western Sydney, Western Sydney and the Blue Mountains, with a strong focus on culturally and linguistically diverse communities.

Crescent Care is seeking a Case Manager to support people with traumatic brain injury and spinal cord injuries, guiding them from hospital discharge to community living.

Qualifications

• Bachelor's degree or higher in eligible health or human services field.
• At least 5 years of full-time experience in person-centred case management for people with severe injuries, disabilities and complex needs.
• Availability for at least 20 hours a week across 4 days during business hours.
• Current Ahpra registration, if your profession requires it.
• A National Police Check, and a Working With Children Check if you will work with people under 18.

• Coordinate rehabilitation, allied health, attendant care and community services.
• Assess risk and put safeguards in place.
• Prepare plans and service requests under the Lifetime Care Guidelines and workers compensation requirements.
• Monitor outcomes and work closely with icare.

About you

• A Bachelor's degree or higher in Occupational Therapy, Social Work, Nursing, Psychology, Physiotherapy, Speech Pathology, Exercise Physiology, Rehabilitation Counselling, or another health, rehabilitation, disability or human services discipline accepted by icare
• At least 5 years of full-time (or equivalent) professional experience in person-centred case management for people with severe injuries, disabilities and complex needs
• Availability for at least 20 hours a week across 4 days during business hours
• Current Ahpra registration, if your profession requires it
• A National Police Check, and a Working With Children Check if you will work with people under 18
