Job summary

Bayside Health, part of Peninsula Health, is seeking a Social Worker to join the Transition Care Program (TCP) supporting older adults transitioning from hospital to home. Based in Frankston, you will perform evidence-based assessment, case management and complex discharge planning in a multidisciplinary team to achieve person-centred outcomes.

Qualifications

• Member or eligibility of the Australian Association of Social Work.
• Current Victorian Drivers license.
• Highly developed interpersonal and communication skills.
• Demonstrated ability to work within a multidisciplinary team.
• Ability to independently deliver high quality social work services, including comprehensive assessment, counselling, patient management, and thorough discharge planning.

Responsibilities

• Limited Term Full Time until October 2027 - 80 hours per fortnight

The Team & Role

Join Bayside Health Peninsula’s Transition Care Program (TCP), part of the Rehabilitation, Ageing, Pain and Palliative Care Services (RAPPS) cluster, supporting older adults as they transition from hospital to home and community-based care across the Mornington Peninsula. As a Social Worker within this multidisciplinary team, you will provide evidence-based assessment, case management and complex discharge planning for a diverse aged care caseload, working closely with clients, families, carers and healthcare professionals to deliver person-centred outcomes. This is an opportunity to contribute to a highly collaborative service that values continuous improvement, clinical excellence and helping older people maintain independence and quality of life in their own homes and communities.

Skills & Experience Required
