Job summary

Bayside Health is seeking a Social Worker to join the Transition Care Program (TCP) within RAPPS in Frankston. You will perform evidence-based assessments, case management, and complex discharge planning for older adults transitioning from hospital to home.

The role requires eligibility with the Australian Association of Social Work, a Victorian Driver's license, and strong multidisciplinary collaboration skills.

Qualifications

• Member or eligibility of the Australian Association of Social Work.
• Current Victorian Driver's license.
• Highly developed interpersonal and communication skills.
• Demonstrated ability to work within a multidisciplinary team.
• Experience in Sub-Acute discharge planning or Case Management.
• Ability to independently deliver high quality social work services, including assessment, counselling, patient management, and discharge planning.

Responsibilities

• Provide evidence-based assessment and case management for a diverse aged care caseload.
• Perform complex discharge planning in collaboration with healthcare teams.
• Engage with clients, families and carers to achieve person-centred outcomes.
• Contribute to continuous improvement and collaboration within RAPPS.

• Limited Term Full Time until October 2027 - 80 hours per fortnight

The Team & Role

Join Bayside Health Peninsula's Transition Care Program (TCP), part of the Rehabilitation, Ageing, Pain and Palliative Care Services (RAPPS) cluster, supporting older adults as they transition from hospital to home and community-based care across the Mornington Peninsula. As a Social Worker within this multidisciplinary team, you will provide evidence-based assessment, case management and complex discharge planning for a diverse aged care caseload, working closely with clients, families, carers and healthcare professionals to deliver person-centred outcomes. This is an opportunity to contribute to a highly collaborative service that values continuous improvement, clinical excellence and helping older people maintain independence and quality of life in their own homes and communities.
