Australian Unity, a leading health and care provider in Australia, seeks a Care Partner Team Leader to steer a Neighbourhood-based team. You will drive care outcomes, lead multidisciplinary teams and ensure services align with customer goals and funding arrangements.

This is a full‑time role based in Mackay across the Northern Region. You will develop Capability, monitor performance, and deliver proactive care management while fostering a culture of accountability and continuous improvement

Qualifications

Responsibilities

• Lead and develop Care Partners, Clinical Care Partners and support team members.
• Drive team performance across outcomes, care planning and engagement.
• Monitor performance data, trends and workload distribution to identify improvements and manage risk.
• Support customers to receive timely reviews, care planning and services aligned to goals and funding.
• Promote effective funding utilization and improved customer outcomes.
• Support governance and quality activities across the team.
• Build relationships across Service Delivery, Scheduling, Clinical Services and Customer Support.
• Foster a culture of accountability and continuous improvement.

We are seeking a Care Partner Team Leader to lead a Neighbourhood-based Care Partnering team focused on delivering exceptional customer outcomes, proactive care management and operational excellence.

In this leadership role, you will drive team performance, develop capability and support the delivery of coordinated services that respond to customers' goals, changing needs and funding arrangements. You'll play an important role in advancing our More Care for Our Customers strategy while contributing to sustainable growth and service excellence.

This is a full-time role based in Mackay looking after the Northern Region including Cairns, Townsville, Rockhampton and Mackay.

About the role

In this role, you will lead a multidisciplinary team and support the delivery of high-quality customer outcomes across your Neighbourhood portfolio.

You will:

• Lead and develop Care Partners, Clinical Care Partners and Care Partner Support team members.
• Drive team performance across customer outcomes, care planning, engagement, retention and operational objectives.
• Monitor performance data, customer trends, utilisation outcomes and workload distribution to identify opportunities for improvement and proactively manage risk.
• Support customers to receive timely reviews, care planning and services aligned to their goals, changing needs and funding arrangements.
• Promote effective funding utilisation and identify opportunities to improve customer outcomes.
• Support risk management, governance and quality activities across the team.
• Build effective working relationships across Service Delivery, Scheduling, Clinical Services, Navigation and Customer Support teams.
• Foster a culture of accountability, continuous improvement and customer focus.

What you'll bring

You are an experienced people leader who enjoys supporting teams, improving customer outcomes and leading operational performance in a fast-paced environment.

You will bring:

• Experience leading teams within aged care, health, community services or another regulated customer-focused environment.
• Demonstrated people leadership capability including coaching, mentoring and developing others.
• A customer-centred approach with a strong focus on quality outcomes and service excellence.
• Ability to use data, insights and performance metrics to drive decision-making and continuous improvement.
• Strong stakeholder engagement and influencing skills.
• Experience working across multiple teams to deliver outcomes.
• Understanding of quality, risk and governance requirements within a regulated environment.
• Ability to lead teams through change and continuous improvement initiatives.
• Knowledge of Support at Home, Home Care Packages or Aged Care Quality Standards, and relevant qualifications in health, aged care, community services, nursing, allied health or business will be highly regarded.
