Job summary

Australian Unity is seeking a Care Partner Team Leader in the Bankstown region to lead a neighbourhood-based team delivering high-quality customer outcomes and proactive care management. You will drive performance, develop capability and support coordinated services aligned with customers' goals and funding arrangements.

The role is full-time based in Bankstown, leading Care Partners across Bankstown, Campbelltown and Moss Vale areas, with a focus on sustainable growth and service excellence.

  • Lead and develop Care Partners and multidisciplinary team members.
  • Drive team performance across outcomes, care planning, engagement and operations.
  • Monitor data, trends and workload to identify improvement opportunities and manage risk.
  • Support timely reviews, care planning and services aligned to customer goals and funding.
  • Promote funding utilization to improve customer outcomes.
  • Support governance and quality activities within the team.
  • Foster relationships with Service Delivery, Scheduling, Clinical Services, Navigation and Customer Support teams.

We are seeking a Care Partner Team Leader to lead a Neighbourhood-based Care Partnering team focused on delivering exceptional customer outcomes, proactive care management and operational excellence.

In this leadership role, you will drive team performance, develop capability and support the delivery of coordinated services that respond to customers' goals, changing needs and funding arrangements. You'll play an important role in advancing our More Care for Our Customers strategy while contributing to sustainable growth and service excellence.

This is a full-time role based in the Bankstown Area. This postion would be leading Care Parnters in the Bankstown, Campbelltown and Moss Vale Areas.

About the role

In this role, you will lead a multidisciplinary team and support the delivery of high-quality customer outcomes across your Neighbourhood portfolio.

You will:

  • Lead and develop Care Partners, Clinical Care Partners and Care Partner Support team members.
  • Drive team performance across customer outcomes, care planning, engagement, retention and operational objectives.
  • Monitor performance data, customer trends, utilisation outcomes and workload distribution to identify opportunities for improvement and proactively manage risk.
  • Support customers to receive timely reviews, care planning and services aligned to their goals, changing needs and funding arrangements.
  • Promote effective funding utilisation and identify opportunities to improve customer outcomes.
  • Support risk management, governance and quality activities across the team.
  • Build effective working relationships across Service Delivery, Scheduling, Clinical Services, Navigation and Customer Support teams.
  • Foster a culture of accountability, continuous improvement and customer focus.
What you'll bring

You are an experienced people leader who enjoys supporting teams, improving customer outcomes and leading operational performance in a fast-paced environment.

You will bring:

  • Experience leading teams within aged care, health, community services or another regulated customer-focused environment.
  • Demonstrated people leadership capability including coaching, mentoring and developing others.
  • A customer-centred approach with a strong focus on quality outcomes and service excellence.
  • Ability to use data, insights and performance metrics to drive decision-making and continuous improvement.
  • Strong stakeholder engagement and influencing skills.
  • Experience working across multiple teams to deliver outcomes.
  • Understanding of quality, risk and governance requirements within a regulated environment.
  • Ability to lead teams through change and continuous improvement initiatives.
  • Knowledge of Support at Home, Home Care Packages or Aged Care Quality Standards, and relevant qualifications in health, aged care, community services, nursing, allied health or business will be highly regarded.
