Job summary

Mercy Health Australia in Young, NSW, seeks a dedicated Care Partner to join our permanent full-time team. You will support clients in the Young region, managing care plans and coordinating Home Care services with a focus on high-quality, person-centred care.

The role requires at least 2 years in aged care case management or disability services, a relevant qualification in community services, and the ability to work autonomously within a collaborative team.

Qualifications

• In depth knowledge of the current Support At Home model of care.
• Minimum 2 years’ experience in aged care case management, aged care services coordination and or disability.
• A relevant qualification in community services or case management that has been attained or currently being acquired.
