Job summary

VMCH is seeking a Care Partner based in Pakenham, supporting clients across Eastern and Southern Metropolitan Melbourne. You will coordinate person-centred services, deliver high-quality care in homes, and manage a growing client portfolio.

This role involves regular travel to metropolitan and regional areas, providing compassionate, client-centred support while partnering with families and communities to improve wellbeing and independence.

Qualifications

• Diploma qualification in a relevant field or higher (e.g. Diploma, Advanced Diploma, Bachelor degree or above)
• Experience in case management, care coordination or client services within aged care, community services or a similar environment
• Experience completing assessments, developing care plans and supporting clients with complex needs
• Ability to manage consumer budgets and support services effectively
• Sound computer skills, including Microsoft Office and client management systems
• Excellent organisational skills with the ability to prioritise and work autonomously
• Understanding of community aged care services, referral pathways and person-centred care principles
• Knowledge of aged care legislation, quality standards and sector reforms
• Current Victorian Driver's Licence and car registration
• Willingness to travel across Metropolitan & Regional areas
• National Police Check

The position

Based in Pakenham, you’ll support clients across Eastern and Southern Metropolitan Melbourne, extending through to Gippsland.
