Job summary

Grand Pacific Health in New South Wales is seeking a Care Navigator for the Caringbah Help aHead service. This full-time fixed-term role collaborates with a team to connect clients to specialist, clinical or community supports and to facilitate timely referrals.

You will work with people and their families, addressing barriers to access, providing information and education, and supporting inclusion of carers.

Qualifications

  • A tertiary qualification in a relevant discipline.
  • Recovery-oriented approach aligned with national frameworks.
  • Understanding of health and wellbeing needs of people with mental health or substance use problems.
  • Ability to engage with consumers and families to provide care navigation and referral support.

Responsibilities

  • Provide person-centred, practical support by connecting consumers with services.
  • Facilitate access to care and ensure linkage to the right services.
  • Collaborate with mental health professionals and external providers to deliver holistic care.
  • Address barriers by sharing information, education, and warm referrals.

About The Role

This is a full-time opportunity, fixed term until the 30th June 2027, with possibility for extension. The Care Navigator, Help aHead Caringbah, works alongside the person to connect to specialist, clinical or community supports as needed. The Care Navigator delivers practical support through information sharing and education, identifying and addressing barriers to access and facilitating warm referrals, ensuring people are linked to the right service at the right time. In partnership with the person, the Worker also explores who else is important or impacted in the person’s life, and supports inclusion of family, carers and other key people where the person chooses. This role may include extended hours as the service operates Monday - Friday, with some extended hours on these days (up to 7pm - two days per week).

Duties

  • Work collaboratively with a team of mental health professionals and external service providers to deliver integrated and holistic care.
  • Address barriers to accessing services by sharing information, providing education, and facilitating warm referrals.
  • Support consumers throughout their journey, acting as a key point of contact.
Skills & Experience
  • A tertiary qualification in a relevant discipline, and/or extensive experience in mental health, health, community services or social sciences.
  • Demonstrated commitment to working from a recovery orientation, in line with the National Framework for Recovery Oriented Mental Health Services.
  • Demonstrated understanding of the health and wellbeing needs of people with mental health and/or substance use problems.
  • Demonstrated ability to engage with consumers and their families or carers to provide care navigation and referral support.
