Calvary Care is seeking a dedicated Care Manager (Full Time) to work in the community, with a base in Mulgrave. You will collaborate with clients, families and stakeholders to deliver person-centred care and manage care plans, budgets and service delivery.

This full-time role requires prior care management experience in Home Care, EN or RN, and a Diploma/Degree in Health or equivalent. A current Australian driver’s licence and reliable vehicle are essential.

Qualifications

• Clear understanding of Support at Home packages and the recent sector changes, strong understanding of the Aged Care Quality Standards and available community service networks.
• Previous or current experience in Care Management within Home Care, EN or RN highly desirable.
• Highly organised with the ability to plan, delegate, and manage competing priorities.
• Current Australian driver’s licence and reliable vehicle.
• Diploma or degree in Health/Allied, or 3+ years' relevant industry experience

About the role

As a Care Manager (Full Time), you will be responsible for collaborating with clients, families and stakeholders to deliver person centred care. You will be based in the community with your clients and join us in the office based in Mulgrave and enjoy free onsite parking
