Job summary

Marathon Health Australia in Narrandera, NSW, is seeking a part-time Care Coordinator to support clients with chronic diseases in collaboration with GPs, nurses, and Aboriginal Medical Organisations.

You will develop tailored care plans, coordinate services, and help improve health outcomes while engaging with Aboriginal and CALD communities. The role emphasizes person-centred care, cultural safety, and professional development.

Qualifications

  • Registration as an Enrolled or Registered Nurse with NMBA and/or as a qualified Aboriginal Health Worker.
  • Experience in assessments, care coordination plans, and evaluating outcomes.
  • Ability to work with Aboriginal and CALD communities and navigate chronic conditions.

Responsibilities

  • Coordinating high-quality care for individuals with chronic diseases and complex conditions.
  • Collaborating with patients, families, carers and the primary care team to implement tailored care plans.

To thrive in this role, you’ll need the following qualifications and skills:
  • Registration as an Enrolled or Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) and/or as a qualified Aboriginal Health Worker.
  • Proven experience in conducting comprehensive assessments, developing care coordination plans, delivering safe and responsive care, and evaluating outcomes.
  • Strong understanding of the complexities involved in caring for individuals with chronic conditions and the ability to navigate these challenges effectively.
  • Excellent communication and interpersonal skills, with the ability to engage and connect with Aboriginal and Torres Strait Islander communities, as well as people from culturally and linguistically diverse (CALD) backgrounds.
  • A commitment to ongoing professional development, ensuring you stay at the top of your game while expanding your expertise and capabilities.
