Job summary
Central Queensland Hospital and Health Service is seeking a Senior Nurse to lead the Introduction of Criteria for Transfer of Care (TICTOC) Cardiac Program at Rockhampton Hospital. Qualifications
- Current Registered Nurse registration with AHPRA (implied by role)
- Advanced knowledge in cardiac nursing practice
- Expertise in evidence-based practice and quality improvement
- Exceptional collaboration and communication with multidisciplinary teams
Responsibilities
- Lead the TICTOC Cardiac Program at Rockhampton Hospital, driving implementation and adoption across sites
- Provide advanced clinical and project leadership, guiding discharge planning and transfer of care
- Drive project delivery, governance, risk management, data analysis and monitoring outcomes
- Embed sustainable change with education resources and implementation strategies to scale the model
