Job summary

Avivo is seeking a Team Facilitator to support a Service Coordinator in Belmont, WA. You’ll lead a team of 10 and assist 50 customers, delivering person-centred care and ensuring high-quality records and plans.

You’ll spend about 80% on direct customer support, with remaining time on administration, mentoring, quality checks, and service coordination. This permanent part-time role requires flexibility across a 7-day roster.

Qualifications

• Certificate III in Disability, Individual Support, Aged Care, Mental Health, or related qualification, or currently working towards completion.
• Flexible availability to work across a 7-day roster, including weekends and varying shift patterns as required.
• Availability to work a minimum of 30 hours per week.
• Current NDIS Worker Screening Check.
• Valid Driver's Licence and access to a reliable vehicle.
• Commitment to high-quality, person-centred support.

Responsibilities

• Provide person-centred support with daily living activities and community participation.
• Deliver support to customers with high-intensity support needs.
• Mentor and guide team members to promote best practice and positive outcomes.
• Work collaboratively with families, guardians, and healthcare professionals.
• Ensure support plans, documentation and records are accurate and up to date.
• Support coordination of services and contribute to a safe, positive team environment.
