Job summary

Optimum Health Solutions is seeking an experienced Behaviour Support Practitioner to join our growing multidisciplinary team. You will conduct behavioural assessments, develop robust Behaviour Support Plans and deliver evidence-based Positive Behaviour Support across clinic, home, community and school settings.

You will work with children, adolescents and adults with complex needs, collaborating with families and support coordinators to drive meaningful outcomes and improve quality of life.

Qualifications

• Current NDIS Behaviour Support Practitioner registration (Core level or above).
• Experience as a Behaviour Support Practitioner or in a similar behavioural support role.
• Strong understanding of Positive Behaviour Support frameworks.
• Experience conducting assessments and developing Behaviour Support Plans.
• Excellent communication and relationship-building skills.
• Ability to work independently while contributing to a multidisciplinary team.

To Be Successful In This Role, You Will Have
