Qualifications

• Flexibility and autonomy in GP practice
• Experience with chronic disease management helpful
• Familiarity with Best Practice software is a plus

Responsibilities

• Provide high-quality medical care within a GP-led clinic
• Participate in multidisciplinary care with nursing, admin and allied health teams
• Contribute to Chronic Disease Management programs
