Job summary

Macedon Ranges Health, supported by Benetas, is seeking an Assessment Officer to deliver in-home HACC-PYP assessments for younger people under 65 in the Gisborne area. You’ll evaluate needs, develop care plans in collaboration with clients and families, and coordinate referrals to Health and Home Care services to support independence.

Based in Gisborne, you’ll join a multidisciplinary team, contribute to consumer directed care, maintain records, and meet program guidelines.

Qualifications

• Tertiary qualifications in Health, Mental Health, Social Work or Social Services.
• Experience completing assessments and care plans in health settings (desirable).
• Experience with younger adults (desirable).
• Current Victorian Driver’s License.
• First Aid & CPR or willingness to obtain.
• Working with Children’s Check or willingness to obtain.
• NDIS Worker Screening Clearance or willingness to obtain.

Responsibilities

• Evaluate the support needs of younger people requiring assistance due to disability, mental health, chronic conditions or short-term health needs.
• Undertake in-home holistic assessments for HACC-PYP clients to maximise quality of life and enablement.
• Identify client goals and care plans in consultation with clients and families.
• Facilitate referrals to Allied Health and Home Care services to achieve goals.
• Undertake intake of referrals within set time frames and maintain client records.
• Monitor targets, quality and compliance requirements per program guidelines.
