Mercy Health Barwon Region is hiring a casual role to conduct client assessments, access respite services, develop services and set up rostering for clients. You will contribute to program growth across CHSP and private client workloads while working within a multi-disciplinary team to deliver flexible, client-centered care.

The role requires resilience, compassion and a commitment to exceptional care, with strong knowledge of CHSP/NDIS and excellent office skills to support daily operations.

Qualifikationen

• Relevant community care experience in intake, assessment and care planning.
• Excellent knowledge of CHSP, NDIS, VHC, DVA, HCPs.
• Excellent time management, organizational and communication skills, with the ability to adapt and problem solve.
• Understanding of the needs of people from special needs groups.
• The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.
• Experience with Microsoft Office suite.

Aufgaben

• Responsible for conducting client assessments, accessing the respite services, service development and rostering set up of clients.
• Support growth of the program, building on the current CHSP program along with growth in private client case loads.
• Review procedures, administration, reporting requirements and community with program stakeholders.
• Be part of a multi-disciplinary team which requires you to provide highly responsive and flexible service to consumers and carers.
