Job summary

Hader Clinic Queensland is seeking a dedicated professional to support residents in a therapeutic community within Gympie, QLD. The role focuses on daily client support, formal assessments, care planning, and group facilitation within a private AOD rehab setting.

You will coordinate with clinical staff, manage referrals, and contribute to psychoeducation sessions while maintaining safety and quality WHS practices.

Qualifications

• Personal lived experience of alcohol/drug addiction and recovery through a 12-step fellowship program.
• Good interpersonal skills and ability to build rapport with clients with substance use and/or dual diagnosis.
• Experience in case management and coordinating care within a therapeutic community setting is desirable.
• Knowledge of legal systems such as bail and parole is advantageous.
• Current driver’s license and ability to commute to Gympie, QLD north of Sunshine Coast.

Responsibilities

• Support and encourage clients in the therapeutic community daily.
• Undertake formal face-to-face assessments of clients, including all necessary paperwork.
• Prepare Care Plans and case manage as directed by Senior Support Worker and Program Coordinator.
• Facilitate and participate in house/group meetings and STEP group sessions.
• Deliver Psychoeducation workshop sessions to clients as required.
• Facilitate client linkages with community and self-help support groups and services.
• Monitor urine analysis results, as needed.
• Coordinate inward and outward referrals under direction from the Program Coordinator.
• Liaise with the clinical team to coordinate care and ensure consistent treatment.
• Assist with facilitation and patient support in Hader Clinic Queensland Private Hospital.
• Participate in staff meetings and work as a team to resolve issues constructively.
