Job summary

Tallangatta Health Service in rural Victoria is seeking a passionate nurse to coordinate and facilitate consumer assessments to support AN-ACC classifications in our residential aged care facilities.

The role requires current AHPRA registration, strong time management and communication skills, and a commitment to person-centred care. We offer a permanent position with flexible patterns to suit the service and the successful applicant.

Qualifications

• Current registration as a Registered or Enrolled Nurse with AHPRA.
• Extensive knowledge and experience with provision of care within an aged care facility.
• Demonstrated ability to perform initial and ongoing assessments for AN-ACC classifications.

Job Description

We are seeking a passionate Nurse to coordinate and facilitate consumer assessments to support AN-ACC classifications and requests for reclassification, as indicated by change in clinical condition of consumers within our Residential Aged Care facilities.

Tallangatta Health Service is a 61 bed, small rural public health service providing a mix of acute, sub-acute, aged care residential, and community services to Tallangatta and the surrounding district. About the Candidate

You are passionate about person-centred care and motivated to enhance the wellbeing of our consumers. You will also meet the following criteria:

• Current registration as a Registered or Enrolled Nurse with AHPRA.
• Extensive knowledge and experience with provision of care within an aged care facility.
• Demonstrated ability to perform initial and ongoing assessments, to support AN-ACC classifications and request/s for reclassification as indicated by change in clinical condition of consumers.
• Demonstrated commitment to person-centred care.
• Well-developed interpersonal and communication skills.
• Strong time management and problem-solving abilities.
• Commitment to continuous improvement.
• Computer literacy with word-processing and data base programs

All appointments are made subject to the required security screening including National Criminal History and an immunisation status appropriate to the role, including the annual Influenza vaccination.
