Amana Living is recruiting a Social Worker to join the Health Care Quality team in Subiaco on a full‑time basis. You will promote psychological safety and well‑being, provide information and support to residents and families, and collaborate with a multidisciplinary team to plan care and coordinate services.

You will undertake psychosocial assessments, assist with discharge planning and provide advocacy, counselling and referrals as residents transition between home, community and residential

Qualifications

• Bachelor of Social Work or equivalent.
• Experience in aged care or social work settings preferred.
• Strong communication and advocacy skills.

Responsabilités

• Undertaking social work assessments in collaboration with the team.
• Liaising with clinical staff and coordinating discharge planning.
• Case management and SAT/guardianship support where appropriate.
• Planning and delivering social work interventions with clients and families.
• Providing care coordination, counselling, advocacy and referrals.
• Supporting nursing and allied health teams with psychosocial care.
