Job summary

Amana Living is seeking a Social Worker to join our Health Care Quality team in Subiaco, WA. This full-time role focuses on safeguarding wellbeing, guiding residents and families through complex care planning and access to community services.

The role requires Bachelor of Social Work, knowledge of aged care, and strong communication, with duties ranging from psychosocial assessments to discharge planning and multidisciplinary collaboration.

Qualifications

• Bachelor of Social Work required.
• Knowledge of the Aged Care Sector.
• Strong time management and prioritisation.
• Excellent problem solving and negotiation skills.

• Exceptional verbal, written and interpersonal communication.

Responsibilities

• Undertaking social work assessments and psychosocial assessments with the team.
• Liaising with clinical staff and informing discharge plans for transport.
• Case management including SAT and guardianship processes.
• Plan and undertake social work interventions with clients and families.
• Provide care coordination, counselling, advocacy and referrals.
• Consult with Nursing and Allied Health on psychosocial care.
• Ensure compliance with Amana Living policies and accreditation.
• Deliver discharge planning for residential or community transitions.
