Qualifikationen

• Bachelor of Social Work is essential.
• Knowledge of the Aged Care Sector is required.
• Strong time management and prioritisation are needed.
• Excellent verbal, written and interpersonal communication skills are essential.
• Evidence of current flu vaccinations is required by policy.
• Ability to obtain a National Police Clearance within 6 months.

Aufgaben

• Undertaking social work assessments in consultation with team members.
• Liaising with clinical staff for discharge planning and transport arrangements.
• Case management including SAT and guardianship processes.
• Planning social work interventions with clients and families.
• Provide care coordination, counselling, advocacy and referrals.
• Support to Nursing and Allied Health regarding psychosocial aspects of care.
• Ensure compliance with Amana Living policies and accreditation standards.
• Discharge planning to home, residential care, or alternate providers.
