Job summary

Access Health and Community seeks a Project Coordinator for the Support at Home Program in Hawthorn, VIC. The role involves coordinating reform projects, engaging with internal and external stakeholders and supporting readiness for aged care program delivery.

You will contribute to service improvement, compliance and better client experiences, reporting to the Service Impact and Development Manager during a max-term position through to 27 June 2027, with a collaborative, values-driven workplace

Qualifications

• Tertiary qualifications in Project Management or Allied Health or related discipline (desirable).
• Proven experience building collaborative relationships with diverse stakeholders to achieve outcomes.
• Experience in Aged Care services or supporting consumers navigating aged care systems.
• Experience in project coordination, service improvement initiatives or organisational change projects.
• Ability to interpret legislative, funding or regulatory changes and develop policies or guidance materials.
• Experience in contract/vendor/client management and reporting systems such as Trak or Power BI.

Responsibilities

• Coordinate the implementation of Support at Home reform projects, ensuring milestones and timelines are achieved.
• Lead project tracking, reporting and documentation activities with recommendations for decisions.
• Build relationships with stakeholders to drive collaboration and outcomes.
• Facilitate workshops and meetings to support readiness and service excellence.
• Monitor compliance requirements and service agreements to align with reforms.
• Analyse service data to identify opportunities for quality improvement and efficiency.
• Support development of policies, tools and resources to strengthen service delivery.
• Manage project documentation and risk registers to ensure timely information.
