Job summary

Northern Sydney Local Health District is hiring for an Aged Care Assessment role based at Macquarie Hospital. You will join a dynamic team of OTs, PTs, SWs and RNs delivering assessments across NSLHD with strong supervision and ongoing education.

The role requires relevant clinical qualifications with full AHPRA registration or eligibility, excellent communication, and ability to work within tight time frames. Aboriginal and/or Torres Strait Islander applicants are encouraged.

Qualifications

• Tertiary qualifications per clinical discipline with AHPRA registration or eligibility.
• Demonstrated ability to undertake comprehensive assessments and develop care plans.
• Experience in aged care settings and working with older persons is desirable.

Responsibilities

• Provide Aged Care Assessment services across NSLHD sites with strong clinical supervision.
• Work with a multidisciplinary team including OT, PT, SW and RN in a fast-paced environment.
• Document assessments and care plans accurately within electronic systems.
