Job summary

NSW Health's Aged Care Assessment Service (ACAS) at Macquarie Hospital is seeking qualified health professionals to join a dynamic team delivering aged care assessments across NSLHD. You will work with OTs, PTs, social workers and RNs to support older clients and their families, within tight timeframes.

Applicants must have relevant tertiary qualifications, full registration or eligibility, and strong communication skills.

Qualifications

• Relevant tertiary qualifications with full AHPRA registration or eligibility.
• Proven high standard of written and verbal communication; timely documentation.
• Experience in aged care settings and ability to develop and implement care plans.

Responsibilities

• Join a dynamic NSLHD ACAS team providing aged care assessments.
• Collaborate with OTs, PTs, SWs and RNs to support older persons and families.
• Demonstrate commitment to high standards within tight timeframes.
