Job summary

The Northern Sydney Local Health District (NSLHD) ACAS invites applications for Permanent Full Time and Part Time positions based at Mona Vale Hospital. You will join a multidisciplinary team delivering aged care assessments across sites, with strong education and clinical supervision provided.

We welcome applicants with relevant tertiary qualifications and full AHPRA registration or eligibility, plus skills in communication and use of electronic patient records.

Qualifications

• Relevant tertiary qualifications in nursing, occupational therapy, social work or physiotherapy.
• Full AHPRA registration or eligibility for membership of the relevant Australian peak professional body.
• Proven ability to undertake comprehensive aged care assessments and develop care plans.
• Strong written and verbal communication with timely documentation.

Responsibilities

• Provide comprehensive aged care assessments across NSLHD sites as part of the ACAS team.
• Deliver high-quality care planning and coordinate with OT, PT, SW, and RN colleagues.
• Contribute to ongoing education and supervision within the team.
