Qualifications

• Current registration as a Registered Nurse with AHPRA and no restrictions.
• Ability to build rapport and maintain solid working relationships with key stakeholders.
• Strong knowledge of the aged care sector and ability to identify trends and best practice.
• Clear thinker with problem-solving skills to convert complex ideas into simple actions.
• Project coordination experience and track record in implementing projects.
• Sound operational knowledge of computerised systems used in the industry and willingness to learn new systems.

Responsibilities

• Provide leadership and direction of the quality of care and experience provided across the home.
• Ensure assessments, care planning and case conferencing are conducted.
• Ensure Care Plans are resident-owned and reviewed after significant changes, with family input.
• Oversee preadmission and admission processes for seamless transitions of care.
• Create and maintain a culture where staff enable residents to make informed care decisions.
• Monitor high-risk residents or those with complex needs.
