Job summary

ACH Group is seeking compassionate Support Workers to provide home care and community support across Adelaide's Southern and Western Suburbs. You will deliver personalised care, assist with daily living, and promote independence in a respectful, customer-centred approach.

You will travel to clients' homes, maintain documentation, and follow care plans while contributing to a supportive team environment. Training and buddy shifts support early success.

Qualifications

• Excellent written and verbal communication skills.
• Ability to travel between customers' homes.
• Administrative and computer skills.
• Ability to operate a smart phone.

Responsibilities

• Provide personalised support to older individuals in accordance with their needs and preferences.
• Assist with activities of daily living, including personal care and domestic tasks.
• Foster an inclusive environment that promotes independence and community engagement.
• Follow and contribute to the customer's care plan.
• Maintain and document care activities with confidentiality.
