Job summary

Dementia and Aged Care Services is seeking an experienced Client Relations Manager to oversee a portfolio of Support at Home clients on the Central Coast, NSW. This office-based role in Kanwal offers autonomy, supportive leadership, and opportunities to impact aging at home.

You will build trusted relationships with clients and families, coordinate quality services, and manage budgets to maximise funding use while ensuring person-centred, wellness-focused care across ~50–60 clients.

Qualifications

• Experience in Support at Home, Home Care Packages or community aged care.
• Background in care management, case management or care coordination.
• Confident managing client budgets and service utilisation.
• Knowledge of person-centred care, wellness and reablement.
• Strong communication, organisation and problem-solving skills.
• Current driver’s licence and access to a reliable vehicle.

Responsibilities

• Manage a portfolio of approximately 50–60 Support at Home clients.
• Develop and review goal-focused care and service plans.
• Coordinate services that promote wellness, independence, reablement and social connection.
• Manage client budgets, expenditure and service utilisation.
• Help clients understand their funding, fees and service options.
• Build strong relationships with clients, families and service providers.
• Monitor service quality and respond promptly to changing needs, risks or concerns.
• Maintain accurate records and meet quality, compliance and reporting requirements.
• Strengthen local referral networks and support service growth.
