Job summary

Silverchain invites a TRANSPAC Care Partner to support older Australians in the North Sydney region, coordinating services and advocating for safe, independent lives at home.

In this 6-month fixed-term role, you’ll combine community client visits with the flexibility to work autonomously from home, working 75 hours per fortnight with hours 8:30am–5:00pm, Monday to Friday, and joining a supportive team with access to clinical, scheduling and service delivery colleagues.

Qualifications

• Experience in community-based case management will be highly regarded.
• Experience supporting clients receiving government-funded aged care services is advantageous.
• Strong communication and relationship-building skills.
• Demonstrated empathy, compassion and a client-centred approach.
• Effective problem-solving and critical thinking skills.
• Well-developed organisational and time management skills.
• Ability to work collaboratively across multidisciplinary teams.
• Advocacy skills to support positive client outcomes.

Responsibilities

• Manage a caseload of older Australians receiving government-funded aged care services in their homes.
• Conduct client assessments and develop personalised Care Plans.
• Monitor and manage client budgets in line with government program requirements.
• Coordinate services to ensure clients receive the right support at the right time.
• Support new clients through onboarding and ongoing service delivery.
• Build strong relationships with clients, families and key stakeholders.
• Work closely with clinical, scheduling and service delivery teams to deliver coordinated care.
• Advocate for client needs and support positive health and wellbeing outcomes.
