Job summary

Healthcare Australia is seeking a proactive Customer Care Coordinator based in Victor Harbor, SA, to guide new and existing home care clients through their aged care journey with empathy, professionalism and expertise.

You will work with nurses, allied health professionals and internal teams to tailor care plans, monitor delivery, and ensure client satisfaction in a hybrid work setting.

Qualifications

• Strong communication and organisational skills with a customer-first approach.
• Ability to build rapport with clients and families and manage stakeholders.

Responsibilities

• Build and maintain relationships with clients and families to understand goals and needs.
• Conduct assessments and develop personalised care plans to support independence and wellbeing.
• Monitor service delivery through regular communication, care plan reviews and client feedback.
• Coordinate with Registered Nurses and Allied Health professionals for complex needs.
• Collaborate with internal teams to ensure services are delivered efficiently.
