Operose Health in Croydon is recruiting an Advanced Nurse Practitioner/Advanced Clinical Practitioner (ANP/ACP) for the Integrated Urgent Care GP HUB. 36 hours part-time positions are available (9-hour days across 1–4 days a week) with up to £60,000 depending on experience and a £5,000 signing bonus.

The role delivers urgent, on-the-day care with GP and AP responsibilities, focusing on history taking, assessment, diagnosis and patient care, aligned with NMC advanced practice and prescriber

Qualifications

• Experienced nurse providing care including initial history taking, clinical assessment, diagnosis and evaluation of care.
• Demonstrates safe, clinical decision-making and expert care for patients with undifferentiated care needs.
• Works with GP Hub team to meet patient needs and provide nurse leadership as required.

The Croydon GP Hubs are in place to provide urgent, on-the-day care, for patients with minor illness or minor injury. We are staffed by a mix of General Practitioners and Advanced Nurse Practitioners, who together provide a wide range of skills, abilities and experience. Main Responsibilities

As a valued member of the Operose Health team, your key duties will include:

• Provide care for presenting patient from initial history taking clinical assessment, diagnosis, treatment, and evaluation of care.
• You must demonstrate safe, clinical decision-making and expert care, including assessment and diagnosis skills for patients within the general practice.
• You will work collaboratively with the general practice team to meet the needs of the patients, supporting delivery of policy and procedures and providing nurse leadership as required.

Ideal Candidate

What we look for in new colleagues:

• The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient including initial history taking, clinical assessment, diagnosis and evaluation of care.
• They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients with undifferentiated care needs.
• The post holder will demonstrate critical thinking in the clinical decision-making process.
• They will work collaboratively with the Clinical Lead and the GP Hub team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
• In order to work at this level, NMC requirement for advanced practice must be met.
• Post holder will be an Independent and Supplementary Prescriber.

Package Description

• Annual salary of up to £68,000 depending on qualifications and experience.
