Job summary

Outcomes Connect Australia (OCA) is seeking an experienced Behaviour Support Practitioner to support people living with disability, focusing on quality of life, reducing behaviours of concern and rights protection. You will work as part of a team with clinical advisors to deliver evidence-based plans.

Endorsement at Core/Proficient/Advanced level and current NDIS registration are essential, with flexible scheduling including remote/community-based work to suit the participant's needs.

Qualifications

• Current registration with the NDIS Commission as a Behaviour Support Practitioner.
• Experience as a Behaviour Support Practitioner for a PBS provider with Core/Proficient/Advanced endorsement.
• Demonstrated ability to work positively in a diverse team.
• Outstanding communication and interpersonal skills.

Responsibilities

• Assess clients (NDIS participants) and plan person-centred supports with families or carers.
• Identify meaningful goals and practical strategies using The Positive Behaviour Support Capability Framework, aiming to reduce restrictive practices.
• Develop, deliver and evaluate evidence-based support plans within a complex support space.
• Ensure individual plans reflect clients' goals, passions and strengths.
• Build the knowledge, capacity and strengths of those who support our clients – family, carers, friends, etc.
