Job summary

Nepean Blue Mountains Local Health District is seeking a Social Worker to join the Acute Care Team at Nepean Hospital. You will deliver high-quality psychosocial assessments and discharge planning to patients and families, supporting adjustment to illness and the hospital journey.

The role emphasises collaboration with a multi-disciplinary team, clear communication, and timely care planning. Prior experience in acute hospital settings is desirable.

Qualifications

• Degree in Social Work from an accredited course.
• Eligibility for membership of the Australian Association of Social Workers.
• Demonstrated understanding of psychosocial impact of hospitalisation.
• Experience in acute hospital settings or medical/rehabilitation settings.

Responsibilities

• Provide high-quality social work services to patients and families across Nepean Hospital.
• Support patients through illness, treatment and hospitalisation.
• Identify and address psychosocial issues affecting transitions and discharge.
