Job summary

Illawarra Shoalhaven Local Health District is seeking an Aboriginal Health Worker/Practitioner - Identified in Warrawong to support Aboriginal people living with chronic conditions through culturally safe, coordinated care.

In the Aboriginal Chronic Care Unit, you will navigate the health system with clients, build trusted relationships and provide person-centred care to improve long-term health outcomes for communities.

Qualifications

• Vaccination requirements for a Category A role.
• Working With Children Check (WWCC).
• Holds a Certificate IV in Aboriginal Primary Health Care (Practice) and a current authority to practice as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA), or holds a minimum Certificate III qualification in Aboriginal Primary Health Care or equivalent.

Responsibilities

• Assist Aboriginal people to navigate the health system and access coordinated care.
• Build trusted relationships with individuals, families and communities.
• Provide culturally safe, person-centred care and empower clients to manage their health closer to home.
• Advocate for clients and coordinate care with other healthcare providers to improve access and outcomes.

As an Aboriginal Health Practitioner/Worker within the Aboriginal Chronic Care Unit, you'll join a small, dedicated team passionate about improving the health and wellbeing of Aboriginal people living with chronic conditions. Working collaboratively with healthcare providers across ISLHD, you'll help Aboriginal people navigate the health system, access coordinated care, and connect with the services and supports they need throughout their healthcare journey. You'll build trusted relationships with individuals, families and communities, providing culturally safe, person-centred care that empowers people to better understand and manage their health closer to home. Through advocacy, care coordination and strong partnerships, you'll play a key role in improving access to healthcare, enhancing patient experiences, and contributing to better long-term health outcomes for Aboriginal communities.

From Helensburgh in the north to North Durras in the south, ISLHD covers an area of 5,687 square kms. That's 5,687 square kms of beautiful beaches, coastal cycling tracks, lush rainforests, incredible bushwalks and friendly people. The region is also host to many annual food, wine and arts festivals and events.
