Job summary

The Royal Flying Doctor Service South Eastern Section (RFDSSE) is seeking an Aboriginal Health Practitioner to deliver culturally safe and clinically supportive care within the KHPMP program across Western NSW, based in Broken Hill. You will assist Aboriginal and Torres Strait Islander clients living with chronic disease to access services and stay engaged in treatment.

• Certificate IV Aboriginal Primary Health Care and AHPRA registration are required.
• NSW Working With Children Check (or ability to obtain) is required.
• Category A vaccinations and National Police Check are mandatory.
• Strong cultural knowledge and connection to community are essential.
• Experience in health or community service delivery is preferred.
• Excellent communication and interpersonal skills are essential.
• Ability and willingness to travel and work in a multidisciplinary team.
• Proficiency with Microsoft Office applications (Outlook, Word, Excel).

About Us:

The Royal Flying Doctor Service South Eastern Section (RFDSSE) is part of one of Australia’s most respected and trusted organisations, providing 24-hour emergency aeromedical retrievals and essential healthcare to people living and working in remote, rural, and regional communities. As the contracted provider for the Western NSW Primary Health Network (PHN), RFDSSE delivers the Keeping Healthy Puura Manti Program (KHPMP) – a time limited initiative designed to improve chronic disease outcomes across the region.

The program strengthens access to culturally appropriate, integrated care by working alongside primary care providers, allied health, and community services to enhance health literacy, self-management, and overall wellbeing.

About the Opportunity:

As an Aboriginal Health Practitioner, you will provide essential culturally safe and clinically supportive care for KHPMP clients, particularly Aboriginal and Torres Strait Islander people living with chronic disease. You will support clients to access services, understand their care, and stay engaged in treatment, while advocating for their needs. This role is offered on a Maximum Term, Full-Time Contract, based in Broken Hill until 30 June 2027.

Key Responsibilities:

• Deliver culturally safe, respectful, and responsive care to Aboriginal clients and communities
• Support access to healthcare services, referrals and chronic disease programs
• Provide health education and promote self-management and health literacy
• Advocate for clients and support informed decision-making about care and treatment
• Work collaboratively with multidisciplinary teams, clinics, and stakeholders
• Support care coordination, including attending appointments where appropriate
• Support and participate in the planning, development and provision of community engagement and health promotion activities
• Maintain accurate clinical documentation and uphold confidentiality and professional standards
• Contribute to continuous improvement and culturally appropriate service delivery
• Certificate IV Aboriginal Primary Health Care (Practice) with current registration as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA)
• Strong cultural knowledge and connection to community
• Experience working in health or community service delivery
• Excellent communication and interpersonal skills
• Ability and willingness to travel
• Ability to work effectively within a multidisciplinary team
• Proficient in Microsoft Office applications, including Outlook
