Darling Downs Health seeks an Aboriginal and Torres Strait Islander Advanced Health Worker to deliver culturally safe, holistic care and support individuals and families across the South Burnett region. You will help navigate health services, advocate for communities and strengthen connections between services and the community.

Competenze

  • Qualification in Aboriginal and Torres Strait Islander Primary Health Care at Certificate IV or Diploma level.
  • Strong understanding of Aboriginal and Torres Strait Islander health, culture and the social determinants that influence wellbeing.
  • Experience providing culturally safe care, advocacy, care coordination and community engagement.
  • Excellent communication skills with the ability to build productive partnerships and mentor others.

What your day could look like

As an Aboriginal and Torres Strait Islander Advanced Health Worker, you'll play a vital role in improving access to culturally safe healthcare for individuals, families and communities. From supporting people through their healthcare journey to strengthening connections between services and community, you'll work alongside multidisciplinary teams to create better health outcomes and lasting impact.

Every day, you'll contribute by:

  • Coordinating care and developing personalised support plans that help people access the right services at the right time.
  • Advocating for Aboriginal and Torres Strait Islander patients and families, helping them navigate healthcare systems with confidence.
  • Delivering health promotion, education and wellbeing initiatives that support informed decision-making and healthier communities.
  • Building strong partnerships across health, government and community organisations to improve service access and continuity of care.
