Job summary

Darling Downs Health is seeking an Aboriginal and Torres Strait Islander Advanced Health Worker to strengthen community ties and improve access to care across the region. You will support navigation of services, advocate for patients, and deliver health education to promote informed choices in partnership with diverse communities.

You will bring cultural knowledge, compassion, and a commitment to improving health outcomes, with opportunities to mentor others and lead community health initiatives

Qualifications

• Qualification in Aboriginal and Torres Strait Islander Primary Health Care at Certificate IV or Diploma level.
• Strong understanding of Aboriginal and Torres Strait Islander health priorities, cultures and communities.
• Experience in care coordination, health promotion, advocacy or community health support.
• Excellent communication skills with the ability to build trusted relationships across diverse groups.
• You'll stand out if you have mentoring experience, leading community health initiatives and building partnerships.
