Job summary

Tamworth Aboriginal Medical Service-Aboriginal Corporation (TAMS-AC), Tamworth site, is seeking a passionate Aged Care Connector to support local Elders and Aboriginal and Torres Strait Islander communities across Tamworth, Manilla, Barraba and Nundle.

You will help people understand their aged care entitlements, connect them with the right services, and navigate the aged care system with confidence, working alongside our Aged Care Support Coordinator to identify local service needs and advocate

• People with Aged Care experience and/or a passion for Aged Care are strongly encouraged to apply.

Tamworth Aboriginal Medical Service-Aboriginal Corporation (TAMS-AC), Tamworth site, is seeking a passionate and community-focused Aged Care Connector to support local Elders, older Aboriginal and Torres Strait islander people and their families across Tamworth, Manilla, Barraba and Nundle.

In this role, you'll help people understand their aged care entitlements, connect them with the right services, and support them to navigate the aged care system with confidence. Working alongside our Aged Care Support Coordinator, you'll also help identify local service needs and advocate for better outcomes for community.

• Comfortable working independently and as part of a team.
