Role: Case Manager - Aged Care (Home Care Packages)

Location: Werribee

Salary: $90,000 per annum + 12% superannuation (negotiable)

Type: Full-time, permanent position

Join a dynamic and growing aged care provider as a Case Manager - Home Care Packages (HCP). This is an exciting opportunity to deliver person-centred care while contributing to the growth of a recently launched aged care program.

Key Responsibilities
  • Provide end-to-end case management for HCP consumers, supporting individuals across VIC, NSW, and QLD, with home visits concentrated in Melbourne.
  • Follow up on leads, conduct assessments, develop care plans, manage budgets, and activate services.
  • Coordinate supports aligned with consumer goals, monitor budgets, complete reviews, and maintain compliance documentation.
  • Ensure adherence to Aged Care Quality Standards and prepare for internal audits.
  • Support consumer onboarding and retention, build relationships with referral sources, and participate in community outreach.
  • Work closely with internal teams to streamline workflows and deliver outstanding consumer outcomes.
Key Skills and Qualifications
  • Minimum 3 years' experience in aged care case management (HCP) or a similar role.
  • Strong knowledge of My Aged Care processes, HCP funding, and Support at Home transition
  • Excellent communication and interpersonal skills
  • Current driver's licence and ability to travel for home visits
