Role: Case Manager - Aged Care (Home Care Packages)

Join a dynamic and growing aged care provider as a Case Manager - Home Care Packages (HCP). This is an exciting opportunity to deliver person-centred care while contributing to the growth of a recently launched aged care program.
Key Responsibilities
- Provide end-to-end case management for HCP consumers, supporting individuals across VIC, NSW, and QLD, with home visits concentrated in Melbourne.
- Follow up on leads, conduct assessments, develop care plans, manage budgets, and activate services.
- Coordinate supports aligned with consumer goals, monitor budgets, complete reviews, and maintain compliance documentation.
- Ensure adherence to Aged Care Quality Standards and prepare for internal audits.
- Support consumer onboarding and retention, build relationships with referral sources, and participate in community outreach.
- Work closely with internal teams to streamline workflows and deliver outstanding consumer outcomes.
Key Skills and Qualifications
- Minimum 3 years' experience in aged care case management (HCP) or a similar role.
- Strong knowledge of My Aged Care processes, HCP funding, and Support at Home transition
- Excellent communication and interpersonal skills
- Current driver's licence and ability to travel for home visits
- Experience with client management systems
- Ideal qualifications - Nursing, Community Services
