Jobbeschreibung
Clinical (RN) Case Manager x 2 positions!

Yes! You may have seen our positions advertised, its not due to staff turnover, but due to business growth!

We are a rapidly growing organisation that is doing extraordinary things for those living with a disability.

We do things a bit differently to other ‘care’ organisations out there. We don’t use agency staff. We build a team of nurses and support workers around our clients to ensure that they have consistency of care, and our employees have regular shifts with the same clients, so they build that relationship.

We have an influx of clients and need an additional Clinical (RN) Case Manager rockstar to join our team! Our clients can be in the complex clinical health space, so we need your clinical expertise!

As a Case Manager you play a vital role in the management of our clients and their supports.

You will be part of our Head Office team based in Southport, working Monday – Friday, 8.00am to 4.06pm. There are around 20 of us in Head Office - we have a wonderful team, who works hard, and you are rewarded and appreciated for your efforts (think coffee & juice runs, birthday celebrations, fun night's out, team lunches, Friday drinks and gift cards!).

Our model works best with you being an excellent RN with solid nursing experience. As the care coordinator/case manager for our complex clients you provide clinical oversight along with mentoring and supporting of our community team of nurses and support staff.

Ideally you have experience with complex health conditions, and you are solid with undertaking comprehensive health assessments, experience with diabetes, catheter management, PEGs and wounds.

Customer service will be important to you, as its important to us! You will have strong relationship management skills and be adept at advocating for your clients whilst liaising with multidisciplinary teams.

The role is a mixture of being in the office and being out in the community visiting clients, training your support team, undertaking assessments and onboarding new clients. There are company vehicles available for use.

We are about finding the right person who has the right attitude, values and work ethic to join our team. We are happy to invest in training and mentoring for the right person! We don’t do KPI’s of billable hours, client satisfaction is our measure of our success!

To be successful you will have:

  • Qualified Registered Nurse with preferably 2-5 years experience
  • Previous case management and/or supervisory experience is desirable, but not essential
  • Ability to work collaboratively in our multidisciplinary team
  • Highly developed verbal and written communication skills
  • Confidence in managing your time effectively including the ability to work autonomously and part of a team
  • IT savvy
  • Blue card and NDIS Workers Screening Clearance or willingness to obtain
  • Full, unrestricted AHPRA registration and legal working rights in Australia
  • Remuneration is paid above the Nurses Award rate of pay
  • Be available to participate in the On Call roster, once every 8-10 weeks (and paid a generous on call allowance). Don’t worry you don’t go on the On Call roster until you are fully settled into your role and are deemed confident!

And you will need to love dogs as we have an office pooch most days that makes us all happy during the day!

We strongly encourage Registered Nurses to apply that may be working in the Hospital or Aged Care industries. If you are looking for a challenge or want to do something fresh, come join an organisation where you are exceptional clinical skills are appreciated!

If this sounds like a position for you, please hit ‘apply now’. Applications will be assessed as they are received.
