As the Client Program Manager (also known as the Manager, Care Planning & Practice), you’ll work closely with the General Manager and multidisciplinary team to oversee day-to-day clinical operations and lead a team of Support Workers. Your role will focus on providing support to our Child Safety and Disability Support clients, ensuring clinical compliance, and driving a positive and supportive team culture.
Responsibilities
- Provide high-quality, professional care management advice and assistance to the General Manager of Operations and DCR Principals.
- Oversee care management for clients who have varying disabilities and/or trauma within the Disability and Child Safety space.
- Write clear care plans for Support Worker implementation.
- Provide regular Support Worker supervision on home visits.
- Ensure all incident reporting is timely, factual and meets the required guidelines for Child Safety and Disability.
- Liaise with other health professionals on client care plans to ensure clients are receiving a high level of individualised support.
- Ensure compliance and safety across SIL housing, support workers, records and documents, processes and regulatory frameworks.
- Complete Client Needs Assessments monthly and as required.
- Participate, on regular rotation, in an after-hours contact roster to provide professional advice to DCR care staff in managing critical incidents and matters affecting clients’ care and support requirements (including critical incident reporting for all emergency events).
Qualifications
- Australian Citizen/ Permanent Resident in Australia
- Bachelor or Master’s Degree in Human Services, Social Work, Special Education, Psychology, or similar field (or comprehensive industry knowledge)
- Demonstrated comprehensive knowledge of legislation, policy and practice in at least one of the following areas: Disability Care, Forensic Disability, Child Protection / Child Safety

- Valid and Current First Aid & CPR
- Working With Children Check (QLD Blue Card) & QLD NDIS Worker Screening Check
- LCS2 Background and History Check
