AdventHealth Shawnee Mission in Kansas is seeking a dedicated Registered Nurse to join a patient-centered care team. You will assess needs, develop care plans with the interdisciplinary team, and deliver education and support to patients and families.

This full-time, day-shift role emphasizes safety, cultural respect, and involvement in improving patient experience through collaborative care and ongoing learning.

Formación

• Associate's degree in Nursing required.
• Bachelor's degree in Nursing preferred.
• RN license required.
• BLS CPR certification required.
• 1+ medical office experience preferred.
• Electronic Health Record experience preferred.

Responsabilidades

• Assess patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data.
• Develop and implement patient care plans in collaboration with the interdisciplinary team.
• Observe outcomes in accordance with nursing standards.
• Provide patient and family education based on assessment of learning readiness, needs, and ability.
• Encourage patient engagement in healthcare through motivational interviewing, self-management, and teach-back.
• Deliver relationship-based care, respecting cultural differences and diversity.
• Delegate tasks to non-RN personnel per policy.
• Coordinate group visits for identified high-risk patient populations.
• Aid in the care transition of high-risk patients, including medication reconciliation and coordination of services.
• Participate in patient experience initiatives.

Job Description:

Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data. Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards. Demonstrates competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection. Advocates for and protects the health, safety, and rights of each patient. Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching. Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology. Delivers relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity. Delegates tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy. Other duties as assigned. Coordinates and implements group visits for identified high-risk patient populations. Aids in the care transition of high-risk patients, including medication reconciliation and coordination of services. Contributes to continuous process improvement by actively participating in patient experience and satisfaction initiatives.
