Job summary

Giving Home Health Care is seeking a compassionate RN or LPN to provide in-home patient care in Alcalde, NM under an established physician care plan. The position is a 1099 contract with a focus on building lasting relationships with patients and families.

Schedule: 9 hours per day, 6am to 3pm, with Monday–Saturday preferred. You will work with CNAs, HHAs, case managers, and physicians to deliver high-quality, compliant care in a home setting.

Qualifications

• Graduate of an accredited college or university with nursing degree (RN/LPn).
• Current State or Compact License as RN or LPN.
• Current CPR certification.
• TB test and physical examination.
• Able to lift up to 50 lbs and travel to patient homes.

Responsibilities

• Execute physician prescribed plans of care.
• Document care provided in the system of record.
• Administer prescribed medications and therapies.
• Conduct patient assessments and coordinate care.
• Assist with activities of daily living.
• Monitor vitals and intake/output.
• Assess skin integrity and provide wound care.
• Communicate health updates with family, physicians, and case managers.
• Educate patient and family on care needs.
• Supervise and collaborate with CNAs and HHAs.
