DaVita Integrated Kidney Care (DaVita IKC) is seeking an RN Case Manager to coordinate and facilitate high-quality ESRD/CKD patient care across a multidisciplinary team. The role includes travel within an assigned territory and field-based work with a home office setup.

You will collaborate with patients, families, physicians, and care teams to optimize clinical outcomes, reduce fragmentation, and support transitions in care while upholding DaVita IKC standards.

Qualifications

• RN degree required; ADN accepted with BSN preferred.
• Current RN license in practice state required.
• Minimum 5 years of clinical nursing experience.
• 3+ years in renal nursing preferred; case management experience required.
• Basic computer skills; Capella proficiency expected within 90 days.

Responsibilities
