Citizens Memorial Hospital (CMH) in Bolivar, MO is hiring a Diabetic Educator to provide diabetes education to patients and others. CMH comprises 34 primary and specialty clinics, 6 long‑term care facilities, and 1 residential care facility, offering acute care, emergency services, ambulatory care, pharmacies, walk‑in clinics, surgeries, addiction recovery, rehab, therapies and more. CMH serves its community throughout the region.

Located in the heart of the Ozarks, our main campus is in Bolivar, Missouri. The area offers affordable housing and local amenities. About the department:

The Diabetic Education Department promotes optimal control of diabetes self‑management skills through education provided to patients and the care team.

A typical day includes:

Providing diabetes education to appropriately referred outpatients and inpatients.

• Performing assessment/data collection in an ongoing and systematic manner focusing on physiological, psychological, and cognitive status of the patient.
• Formulating or following a goal‑directed plan of care which is prioritized and based on the medical diagnosis and intended patient outcomes.
• Documenting patient progress toward meeting goals and making revisions as needed.
• Individualizing instruction with consideration for social, economic and cultural uniqueness of each patient.

Job Requirements:

• Education: Bachelor’s Degree in Dietetics from an accredited university and completion of an approved internship or qualifying experience.
• Experience: Minimum of one (1) year of experience in diabetes self‑management education.
• Skills: Organization, ability to work in a fast‑paced environment, multitasking.
• Licensure & Certifications: Registration from the Commission of Dietetic Registration; Licensed Dietitian in the state of Missouri; or Registered Nurse licensed in the state of Missouri.
• Physical: Ability to be mobile and to stand for extended periods of time.

The position is: PRN, rotating, hours may vary.
