Job description

Job Overview:
The Assistant Nurse (Home Care) plays a vital role in delivering personalized care to individuals who require assistance with daily activities within their own homes. This position is focused on supporting clients in maintaining their physical, emotional, and mental well-being, while fostering an environment that promotes independence, dignity, and comfort. The Assistant Nurse will collaborate closely with clients, families, and other healthcare professionals to ensure high-quality care and a positive experience in the home care setting.

Key Responsibilities:

  • Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, feeding, and mobility support.
  • Monitor clients' health conditions by checking vital signs, administering prescribed medications, and reporting any changes to senior nursing staff or physicians.
  • Provide emotional support and companionship, helping clients manage feelings of loneliness, anxiety, or depression.
  • Help with the coordination of healthcare services, including scheduling appointments, arranging transportation, and liaising with doctors or other healthcare providers.
  • Maintain a clean and safe home environment by performing light housekeeping tasks, including tidying up living spaces, laundry, and meal preparation.
  • Educate clients and their families on proper care routines, health management, and the use of medical equipment as necessary.
  • Document client care and progress, maintaining accurate and up-to-date records.
  • Follow established care plans and participate in care plan reviews to ensure that client needs are being met.

Qualifications:

  • Certification as a Nursing Assistant or Home Health Aide (HHA) is preferred.
  • Previous experience in a home care or healthcare setting is an advantage.
  • Strong communication and interpersonal skills, with the ability to develop trust and build relationships with clients and their families.
  • Compassionate, patient, and empathetic approach to caregiving.
  • Ability to work independently and as part of a team.
  • Basic knowledge of medical terminology and healthcare procedures.
  • Ability to lift and assist clients with mobility or other physical tasks.

Working Conditions:

  • Full-time or part-time hours depending on client needs.
  • Flexibility to work evenings, weekends, or holidays as required.
  • Travel to clients’ homes within a designated area.
